When you register for 4-H Ontario, we collect personal information to help you participate in the program, keep everyone safe, and stay in touch provincially and locally. We'll use this information to send you updates about 4-H events and activities. Your details may also be used for program improvement and required reporting. We protect your privacy and share your information with others only when absolutely necessary for legal reasons or your safety.

Cloverbud participants must be 6-8 years old prior to January 1st, 2025.
Youth Member participants must be ages 9-21 prior to January 1st, 2025.
If you enter a date and receive an error message, or the birthdate is greyed out, this means that the participant is not eligible for participation.

Your Home Association is the local 4-H chapter where you prefer to be engaged in programming.  Often, participants choose to register in the Association closest to where they live. I consent to the collection of personal health information as contained in this form.
By not consenting to the collection of personal health information, the participant understands that 4-H Ontario staff and volunteers will not be provided with the information needed to respond in the event of a health and safety emergency.

4-H Ontario Media Release 

While participating in 4-H activities, photos and videos of participants will be taken. These photos are typically used to record memories and promote 4-H. Please indicate below your consent to the use and reproduction by 4-H Ontario (representing the Ontario 4-H Council and Ontario 4-H Foundation) of any and all photographs, video recordings and audio recordings taken of myself or my child for the use on 4-H websites, in print and other media for the purposes of promotion, illustration, advertising or publication and without compensation.

All such photographs, video recordings, audio recordings and all recorded media, prints and created media from the content shall constitute the property of 4-H. In addition, the undersigned hereby consents to authorize the publication of the name of the participant.

Important Reminder: Photos, images and media may appear in electronic form on the Internet or in other publications outside of 4-H Ontario’s control.

Contagious Illness Acknowledgement
For participants 18 years of age or older, as well as parents/guardians:

I, the undersigned hereby acknowledge the contagious nature of certain illnesses (known and unknown) and acknowledge that there is no participant accident or liability insurance coverage for infectious diseases and assume the risk of potential exposure or infection while participating in 4-H activities.

By signing below, the participant and parent/guardian (if youth is under 18 years of age) hereby acknowledge that they have thoroughly reviewed the documents  above and agree to adhere to the guidelines and terms outlined within them.
