4-H Ontario Outstanding New Volunteer Award Nomination Form

This award is designed to honour new 4-H Ontario volunteers who embody the 4-H pledge. The award celebrates those volunteers who have been with 4-H Ontario for five years or less. Nominations for this award will be accepted across the province. Only one nomination will be accepted per 4-H Association, with one award given in each region. Therefore, a total of six awards will be presented at local 4-H Awards programs in the association where the selected recipient volunteers. Individuals can only receive this award once.
